Epilepsy Passport

The Epilepsy Passport contains essential up-to-date information about a child or young person’s epilepsy, including their emergency care plan, medication history and key professional contacts.This has been developed by the Royal College of Paediatrics and Child Health (RCPCH) with input from key epilepsy professionals, parents, children and young people and funded by the Healthcare Quality Improvement Partnership (HQIP).
