Information for bereaved families after someone dies in NHS care

Information for families following a bereavement has been prepared with the support of families, NHS trusts and other stakeholders. These documents aim to explain what happens next, including information about how to comment on the care your loved one received and what happens if a death will be looked into by a coroner:

I think these resources are really good, however I am concerned that they do not mention the LeDeR programme which all deaths of a person with a learning disability will be part of. It would be great if LeDeR could be incorporated so that people are aware of it so they are prepared when someone contacts them.
