We are currently looking to recruit a number of posts including coordinators, and a trainer to provide long term support for people with a learning disability and behaviour that challenges services. This is one of two new initiatives as part of the transforming Care agenda.
The service will ensure consistent and effective long-term support is available for people with challenging and complex needs and their carers by:
This service will be an integral part of our Community Assessment and Intervention Service and will work closely with the Transforming Care team and other services in Hertfordshire County Council.
The Post will be based at one of the Team bases which are:
• Saffron Ground, Ditchmore Lane, Stevenage, SG1 3LJ
• Rosanne House, Parkway, Welwyn Garden City, AL8 6HG
• St Paul’s, Slippers Hill off Allandale, Hemel Hempstead, HP2 5XY
• Colne House 21 Upton Road, Watford, WD18 0JL
Some travel by car throughout the area covered by the service will be required.
