Making an emergency plan

Just to give an update on the resources for making an emergency plan. The information on how to make an emergency plan and the template for writing down information have now been put together into one document which can be found here . There are also a few templates in the PDF to help gather information about a person’s support needs should there be a need for a change in support arrangements.

There are 2 versions. One is for reading on-line, printing and/or filling in by hand. The other is a version with text boxes that can be filled in on a computer, saved and sent to others (this could be for other people to add their ideas or to make sure they know about the emergency plan). Family carers, who don’t have a computer or would find it difficult to complete, could be supported over the phone to talk about what they would like to put in the plan and it could be filled in by the supporter and, with permission, shared with relevant people.
The templates can be adapted to the needs of people using them- they are just a tool to help people think through their worries and put some essential things in place.
