Digital record keeping

My son’s care provider record service user’s details in paper format which is archived at the end of the month. This means that health issues and other information cannot be easily accessed by the support staff on my son’s team. I want to get him a laptop where his care plan, menu plan, health, hospital and other information can be stored and updated as and when required and can also be quickly accessed. Does anyone have any ideas on the best way to document this information on a laptop or know of any templates I can access to set it up?
Thank you,

Hi Tillypop

Seems like you need some sort of app that you can update as needed where you could include relevant information to share.

You could have a look at https://www.hearmenowapp.com/

The Rix centre has developed some helpful software and might be able to give more advice and support

https://rixresearchandmedia.org/software/

There are some more specific hospital passport apps:

https://apps.apple.com/gb/app/hospital-passport/id1624560627

Here is an editable hospital passport document:

https://www.togetherforshortlives.org.uk/resource/editable-hospital-passport-template/

I am sure there are other resources out there!

Hi Tillypop
This sounds like a great idea.
If I was you I would start with a conversation with your son’s support provider.
I’m guessing it’s going to be most useful to you and your son and his supporters if it fits with the providers paperwork and systems. The easiest it is for them to use, the greater the chance of them doing it?
And of course, share with them any examples you like too.

Good luck

Hello Debora, thank you so much for this information, I’ll check it out. My son already has a hospital passport but I’ll compare it to the download.
Best wishes
